How it works

Six phases. Every patient, on their own clock.

Each phase has its own message cadence, reply gates, and behavioral triggers. The engine advances patients automatically - no coordinator involvement required.

Phase-aware progression

Six clinical phases - Initiation, Onboarding, Activation, Momentum, Plateau, Maintenance - each with their own message cadence, voice, and reply gates.

Trigger-driven outreach

Behavioral predicates evaluated every 60 seconds: drift after 48 hours, escalation after 5 days, plateau acknowledgement, phase advancement. Same trigger never fires twice in a day.

Closed-loop correction

When a patient drifts, the engine detects the pattern, sends a targeted correction, and tracks the response. Auto-resolves on reply. Escalates to the clinic if silence crosses the threshold - no manual monitoring required.

Patient journey

Every patient progresses on their own clock. The engine schedules against the patient’s timezone, gates on replies, and advances automatically when a phase completes.

The patient experience

What arrives on the patient’s phone

Short, direct, timed to their phase. Not a blast - a behavioral cue that arrives when the patient needs it, with a simple reply gate before anything advances.

  • Sent at the patient’s local time, not server time
  • Waits for a reply before moving to the next step
  • Flags the clinic when a patient goes quiet

Hi Marcus - Day 3 of your program. How did your first injection go? Reply YES if you’re on track.
YES
Great. Your next dose window opens in 6 days. We’ll check in then - keep it up.

Trigger logic

Trigger-driven, not schedule-driven.

Every trigger evaluates every 60 seconds, deduped per patient per day. Same trigger never fires twice in a single day for the same patient.

Drift detectedno_response_48h
48 hours without reply

Schedule a phase-appropriate nudge message automatically. No coordinator required.

Escalationno_response_5d
5 days without reply

Flag the patient and alert the clinic by email. One summary per clinic per tick - not a flood of individual alerts.

Phase advancephase_auto_advance
On phase duration completion

Automatically move the patient to the next phase, reset the phase clock, and schedule the opening message set.

Manual overridemanual_flag
Coordinator-triggered at any time

Flag any patient for human follow-up at any point in the journey. Auto-unflag when the patient replies.

Drift Correction

Beyond triggers - the loop that closes itself.

When a patient drifts, the engine identifies the specific behavioral pattern - side effect, missed dose, withdrawal, plateau - sends a targeted correction, and tracks whether they come back. Auto-resolves on reply. Escalates to the clinic if they don’t.

Why SMS alone isn’t enough

SMS reminders aren’t engagement. Behavior is.

Generic reminder apps blast messages on a schedule and call it a retention strategy. Adherix reads what the patient does - and doesn’t - and responds accordingly.

Generic SMS reminder app

Schedule, not behavior

  • Same message every day on the same schedule
  • No phase awareness - week 1 looks like week 12
  • Treats every reply (or non-reply) the same
  • No clinic routing when a human needs to step in
  • Engagement metric is "did the message send"
Adherix behavioral engine

Phase-aware, trigger-driven, clinic-routed

  • Six-phase progression: Initiation → Onboarding → Activation → Momentum → Plateau → Maintenance
  • Trigger-driven outreach: drift detection, plateau, manual flag
  • Reply-gated advancement — won’t move on until the patient confirms
  • Behavioral event log feeds analytics and clinic routing
  • Engagement metric is “did the patient stay enrolled”

Built on the same SMS rails. Different engine.

Clinic dashboard

One view. Every patient. Where they are, and what’s next.

The clinic dashboard surfaces drift before it becomes churn. Coordinators see who needs a call, who’s on rails, and what the engine has already done on their behalf.
